Fall Open

20 branching simulations — 16 accredited for CME credit. Hidden conditions, real management decisions. Earn Faculty Honor, Top Champion, and State Champion.

What you compete for

National recognition. Earned, not assigned.

No nominations and no institutional gatekeeping. The leaderboard reflects the decisions you make inside the same clinical challenges as oncologists nationwide.

National · #1

Champion of US Oncology 2026

Top performer in the Championship Finals, announced live February 3. The inaugural national title for US community oncologists.

National · Top 20

National Frontline

The physicians who first see, screen, and route the hard cases. Top 20 nationally for 2026.

State · #1

State Champion of Oncology 2026

Top performer in each state with at least 10 participants. State Frontline awarded in states with deeper fields.

Great oncology care is built decision by decision.

Every case asks you to recognize risk, reduce uncertainty, and act at the right time.
Your score reflects the full pathway, not a single multiple-choice answer.

01

Spot the patterns that should raise concern for cancer and distinguish them from common mimics.

02

Choose imaging, biopsy strategy, staging, and biomarker testing in the right sequence.

03

Use clinical, radiologic, pathologic, and molecular clues to separate competing diagnoses.

04

Make evidence-based treatment decisions, rate your confidence, and see how both affect your standing.

How you compete

Two ways to compete.

The simulator is the core of the program — duels put your reasoning to the test head-to-head.

Accredited for CME

Clinical Simulator

Work a full patient case the way you would in clinic, with feedback at every step.

Head-to-head

Duel

Challenge another oncologist to ten focused questions and earn points for the leaderboard.

Points from cases and duels rank you on national and state leaderboards. CME credit is earned only through the accredited simulations — duels and the leaderboard carry no separate credit.

FAQ

Questions, answered.

The practical details before you start your first case.

Yes. No fee, no membership, no payment details. It's open to any US oncologist with an active NPI. A one-time NPI check applies only when you enter the ranked championship; the Opening Case needs no account at all.

Up to 15 minutes per simulation. You work through cases at your own pace, one sitting or many.

Up to 4 hours total for the accredited cases in a season. You can earn 0.25 CME credits per accredited case, up to 4.0 per season.

Yes. The simulations run in your browser on phone, tablet, or desktop. There's no app to install.

That's fine. Drop in and out at any point. Every credit you claim is yours to keep, whether you complete one case or all of them.

Your decisions are benchmarked anonymously against the national field. Everyone competes anonymously under a pseudonym; your real name and affiliation are never shown to anyone, including other participants.

Upon completion of this activity, participants should be able to:

  • Identify the clinical and pathologic features (age at onset of 46 years or younger, bilateral or multifocal disease, non-clear cell histology, synchronous or metachronous extrarenal lesions, relevant family history) that warrant referral for genetic counseling and germline evaluation rather than continued management as sporadic disease.
  • Determine the appropriate next diagnostic action when a hereditary cancer syndrome is suspected, including germline panel testing, referral for genetic counseling, cascade testing of at-risk relatives, and initiation of guideline-based surveillance while specifying the surveillance interval and imaging modality indicated for a patient with a confirmed syndrome.
  • Differentiate immune-related adverse events from disease progression, infection, and other causes to guide CTCAE grading and grade-appropriate management, including continuation, suspension, or permanent discontinuation of checkpoint inhibitor therapy, corticosteroid initiation and taper, and referral for subspecialty evaluation.

This activity is developed and delivered with AI assistance. Some cases are developed entirely by faculty without AI. Where AI is used, it assists faculty, who set and validate the correct clinical pathway for every case. All AI-assisted content is reviewed by qualified faculty reviewers to confirm relevance, scientific accuracy, and alignment with the educational scope of the activity. The activity is AI-assisted, not AI-generated. The AI tool used is Claude (Anthropic), versions Opus 4.8 and Opus 5, used August 2026 through September 2026. Prompts and participant responses processed by AI are not stored externally.

How AI is used
  • Drafting. AI helps draft candidate cases and decision pathways for some cases. Faculty set and validate the correct clinical pathway for every case, and qualified reviewers confirm each case for relevance, scientific accuracy, and fit with the educational scope.
  • Aggregate analysis. AI performs de-identified, aggregate analysis of participant decisions at each decision node, including clustering of common errors, for educational-outcomes reporting and the design of future activities. It does not alter content for any individual during the activity.
  • Identity and integrity verification. AI flags possible duplicate registrations and shared responses for human review. AI does not make registration or credit decisions. Registration is verified by the platform, with staff reviewing any flagged cases. Credit is awarded under the accreditation of Partners, the accredited provider.
Validation and oversight
  • Each AI-assisted case is clinically validated by faculty before release.
  • Every case passes two independent review rounds conducted by different reviewers.
  • The full case mix has been reviewed and approved by faculty.
  • Faculty reviewers have no relevant financial relationships and were screened for the potential to insert commercial bias.
  • Faculty reviewers can recommend revision of any AI-assisted content, and Partners, as the accredited provider, can revise or discontinue it.
Independence and responsibility
  • Partners for Advancing Clinical Education is the accredited provider and has reviewed the educational content for accuracy, timeliness, fair balance, and to ensure that it supports safe, effective patient care.
  • AI does not autonomously determine content, results, or credit.
  • No commercial supporter had any role in content design, case selection, scoring methodology, or recognition.
